CHORUS COMMUNITY HEALTH PLANS, an insurance program previously administered as Together with CCHP, provides vision benefits for children 18 and younger utilizing our list of providers and locations. Benefits include, one routine eye exam and lenses (glasses or contacts) per benefit year, and one pair of eyeglass frames from the Pediatric Eyewear collection every two years.  P.V.S. is unable to schedule vision screenings for children 5 or under. Please follow up with your primary care physician in those instances. Professional Vision Services is pleased to offer enhanced P.V.S. Card benefits for the non-covered adult family members of Chorus Community Health Plans.  This discount card is accepted at all the same locations and providers as your children's benefit.  Please indicate that you are a Chorus Community Health Plans family when you enroll for your P.V.S. Card.
